About the Role

As a Social Worker in CMHTOA, you will:

  • Support vulnerable residents to live independently

  • Build lasting relationships with those you support

  • Work in partnership with carers, health professionals, and community services

  • Help deliver seamless, person-centred support

  • Embrace diversity with empathy and respect

Key Responsibilities

  • Provide support to individuals with diagnosed memory problems and mental health needs

  • Coordinate short-term interventions and long-term support

  • Complete strength-based assessments and develop person-centred care plans

  • Handle safeguarding referrals and conduct risk assessments and investigations

  • Collaborate with internal and external teams to ensure high standards of care

Candidate Requirements

You should have:

  • Registration with Social Work England

  • Experience working in the current Social Care System and Framework
