As a Social Worker in CMHTOA, you will:

Key Responsibilities
- 
Provide support to individuals with diagnosed memory problems and mental health needs

Coordinate short-term interventions and long-term support
 - 
Complete strength-based assessments and develop person-centred care plans
 - 
Handle safeguarding referrals and conduct risk assessments and investigations
 - 
Collaborate with internal and external teams to ensure high standards of care

Registration with Social Work England

Experience working in the current Social Care System and Framework
