The Enhanced Primary Mental Health Service (EPMHS) provides a multidisciplinary service to individuals with moderate to severe mental health difficulties in primary care. It focuses on psychological support for those who do not meet criteria for Adult Community Mental Health Services or NHS Talking Therapies Teams. Service users often face multiple social stressors.

The Social Worker will develop and lead this role, working directly with those with complex difficulties, supervising support staff, and advising the wider multi-disciplinary team (MDT) on social care matters.

You will provide assessment, care planning, and social care interventions to adults experiencing mental ill-health, delivering recovery-focused care emphasizing choice, control, independence, and safety. You will champion social care and safeguarding locally.

Main Duties of the Job

  • Assess needs and outcomes; analyse complex information regarding service users and carers.

  • Provide professional accountability by making recommendations based on analyses.

  • Involve service users and carers to learn from their experiences.

  • Undertake surveys and audits locally and contribute to Trust-wide surveys.

  • Deliver social care assessments and manage commissioned care packages.

  • Ensure compliance with statutory social care duties as per the Care Act 2014.

Job Description & Responsibilities

Key Relationships

  • Work as part of a multi-disciplinary team.

  • Collaborate with service users, carers, and stakeholders inside and outside the Trust.

  • Promote dignity, respect, personalisation, choice, and control.

  • Act as a champion and expert in social care and safeguarding.

  • Ensure colleagues deliver high-quality social care services.

Duties and Responsibilities

  1. Champion social care with deep knowledge of legislation and statutory guidance.

  2. Lead adult safeguarding practice, undertake enquiries, and support care plans protecting adults and children from abuse.

  3. Conduct comprehensive assessments for people with high risk or complex social/legal/environmental needs, ensuring eligibility under The Care Act 2014.

  4. Provide direct support and signposting to service users and carers who do not meet social care eligibility.

  5. Ensure commissioning of health and social care services, including offering direct payments and managing charging processes.

  6. Contribute to EPMHS core tasks including Dialog+ First Conversations.

  7. Promote choice and community living with appropriate support and access to universal services.

  8. Perform regular risk assessments to protect vulnerable individuals.

  9. Conduct carer assessments, contingency plans, and support provision.

  10. Facilitate recovery-focused community services and share best practices locally.

  11. Involve service users and carers in continuous service improvement.

  12. Maintain accurate and up-to-date computer records.

  13. Pursue post-qualification and ongoing training to maintain Social Work England registration.

  14. Provide management and reflective supervision to newly qualified social workers and unregistered staff.

  15. Support newly qualified social workers through ASYE and consider training as an ASYE assessor.

  16. Undertake or train for Approved Mental Health Professional (AMHP) duties.

  17. Participate in supervision and reflect on professional practice as a social worker and AMHP.

  18. Fulfill delegated social care responsibilities on behalf of Hertfordshire County Council.

  19. Comply with Trust policies and procedures.

  20. Complete additional training as required.

  21. Perform any other duties assigned by your line manager.

Qualifications / Education / Training

  • Professional social work qualification (essential)

  • Degree-level education (essential)

  • Registration with Social Work England (essential)

  • Training in outcomes-focused assessments and personalisation (essential)

  • Relevant post-qualifying social work training (essential)

  • Approved Mental Health Professional/Best Interests Assessment training (essential)

  • Safeguarding training and experience (essential)

Knowledge, Training, and Experience

  • Minimum 2 years post-qualification experience (essential)

  • Knowledge of relevant legislation, theory, and policy (essential)

  • Experience in assessments, personalisation, choice, control, and care planning (essential)

  • Understanding of personal budgets and direct payments (essential)

  • Experience in multidisciplinary and partnership working with service users and carers (essential)

  • Experience as AMHP or Best Interests Assessor (desirable)

  • Experience working in integrated health and social care (desirable)
