Job Summary

The Social Care team at Torbay and South Devon NHS Foundation Trust is looking for an enthusiastic and experienced Social Worker to join their Baywide area team. This role offers the opportunity to make a positive impact through holistic social care assessments, working in partnership with service users, families, and carers.

The Baywide Social Care Team is a small, supportive group passionate about delivering quality care. They mentor team members and students, promoting a healthy work/life balance.

Main Duties of the Job

  • Be a core member of the multi-disciplinary Baywide Social Care Team.

  • Contribute to effective assessment and discharge planning with working age adults in Torbay and for Torbay residents in out-of-area hospitals.

  • Undertake holistic social care assessments in partnership with service users, families, and carers.

  • Prepare care plans and commission services to deliver cost-effective care packages.

  • Ensure compliance with statutory requirements and Trust policies.

  • Safeguard adults and children, operating within Human Rights legislation.

  • Practice using social care legislation, including Care Act 2014 and Mental Capacity Act 2005.

Job Responsibilities

Communication and Working Relationships

  • Manage communication with people facing complex emotional, psychological conditions.

  • Use skills such as anger management, motivation, negotiation, counselling, and empathy.

Planning and Organisation

  • Plan and prioritise own caseload.

  • Draw up detailed care plans with service users and carers.

  • Participate in referral allocation or self-allocate work as per team practice.

Responsibility and Accountability

  • Manage a defined caseload and provide statutory Social Work services.

  • Complete high-quality assessments and reviews per Care Act 2014 guidelines.

  • Promote service user participation in support planning with transparency.

  • Collaborate with Integrated Mental Health Teams, Adult Social Care, Health services, Police, Education services, and providers.

  • Advocate for vulnerable people and lead safeguarding enquiries as needed.

  • Balance risk management with respect for self-determination, managing complex and sometimes volatile situations.

  • Lone working may be required.

Policy and Service Responsibility

  • Complete Care Act assessments and care plan records following service policies.

  • Work as part of integrated health and social care teams.

  • Contribute to service development within the Hospital Discharge Team.

Finance, Equipment, and Resources

  • Work creatively with communities and voluntary organizations to arrange support plans within resources.

  • Report service provision deficiencies to Social Care lead.

Supervision, Leadership, and Management

  • Participate in regular supervision with line manager.

  • Identify development needs and attend relevant training.

  • Agree annual training and development plans.

IT and Administrative Duties

  • Maintain accurate records of work.

  • Complete social care panel paperwork when required.

Person Specification

Qualifications and Training

  • Essential: Relevant professional Social Work qualification and registration with Social Work England.

  • Desirable: Post-qualification training and consolidation.

Knowledge and Experience

  • Safeguarding adults, Care Act, Mental Capacity Act knowledge.

  • Ability to role model good practice, provide supervision, and hospital discharge planning advice.

  • Managing expectations and jointly working complex cases.

  • IT skills for care management systems.

  • Ability to communicate well verbally and in writing.

  • Commitment to anti-discriminatory practice and equal opportunities.

Desirable Skills

  • Court work experience (Court of Protection, Coroner’s Court).

  • Experience with Paris health and social care recording system.

  • Knowledge of Health and Safety, risk assessments, Continuing Care, Direct Payments, Intermediate Care, and end-of-life care.

Specific Skills

  • Experience in mental capacity assessments, safeguarding, hospital discharge, and best interest decision-making.

  • Experience with court work and end-of-life care is desirable.
