Introduction to the Role

Livewell Southwest is seeking a qualified and experienced Social Worker to join the South East Locality team on a permanent, part-time basis (32 hours per week). Main Responsibilities

Social Care Assessment and Review

  • Complete Care Act Assessments and Reviews to determine eligibility for services

  • Conduct Mental Capacity Act assessments

  • Coordinate ongoing support planning in collaboration with service users and families

  • Apply principles of person-centred care in decision-making and risk management

Safeguarding and Legal Responsibilities

  • Carry out safeguarding investigations for vulnerable adults

  • Prepare legal documentation for court proceedings

  • Represent the organisation in court when necessary

  • Make and support decisions in complex situations involving potential deprivation of liberty

Multidisciplinary Collaboration

  • Liaise with health professionals, local authorities, and partner agencies

  • Engage in joint care planning across multidisciplinary teams

  • Contribute to integrated care pathways and community-based support structures

Professional Development

  • Maintain professional registration with Social Work England

  • Participate in training, team supervision, and protected CPD activities

  • Work towards and/or maintain Post-Qualifying Awards in Social Work

  • Adhere to the Professional Capabilities Framework and uphold the Social Work Code of Ethics


Required Qualifications and Registration

  • Degree in Social Work or equivalent (e.g., DipSW, CQSW, CSS)

  • Active registration with Social Work England

  • Commitment to continuous professional development and reflective practice


Experience and Knowledge Requirements

Essential Knowledge

  • Sound understanding of The Care Act 2014 and Mental Capacity Act 2005

  • Familiarity with the Human Rights Act, Mental Health Act (1983, amended), and Children Act (1989)

  • Experience conducting assessments in line with NHS Continuing Healthcare eligibility criteria

  • Working knowledge of safeguarding legislation for both adults and children

Desirable Experience

  • Prior experience supporting individuals with complex needs, including physical disabilities, mental health issues, or chronic illness

  • Understanding of the effects of caregiving and ability to support carers

  • Previous involvement in the planning and delivery of personal budgets and care packages

  • Familiarity with multi-agency frameworks and inter-professional collaboration


Technical Skills and Competencies

  • Proficient in Microsoft Office Suite for report writing, assessments, and data entry

  • Ability to manage a caseload and prioritise competing demands effectively

  • Strong analytical and decision-making abilities, especially in high-risk or crisis scenarios

  • Excellent communication and negotiation skills to work with service users, families, and professionals

  • Competence in observing, documenting, and assessing client needs in their home environment

  • Confident in contributing to team goals and service performance objectives


Band and Pay Structure

This role is offered at NHS Band 6, with a salary range of £37,338 to £44,962 per annum, pro-rata. The position includes NHS pension contributions, annual leave entitlement, and access to additional employee benefits.
